Hippo Manager offers a variety of pre-made Client Forms, which can be via a widget on the Choose Action screen. If you don't see this widget available, you may need to modify your user settings. For more information, check out Choose Action: Customizing Choose Action Settings to Optimize Workflow.
From the Choose Action screen, select the Client Forms widget which will display all available client forms in your platform.
After you've selected the icon to view the form, you can download and/or print the form using the buttons in the upper right corner of the page.
If your practice has existing forms that you prefer to continue using, they can be added to your platform using the Custom Forms feature. For more information, check out Administration: Creating and Using Custom Forms.
